Hello, if you have been instructed by you case managment agency to get fingerprints, you will need to gather the following documents, then CLICK HERE to schedule online or call 863-519-8900 x281 to make an appointment. Out of respect of our clients' time, we do not take walk-ins.

Schedule Appointment

Currently, we are scheduling appointments on Monday, Wednesday and Friday mornings as well as Tuesday and Thursday afternoons. Please allow 10-20 minutes per person for fingerprints.

You will need the following items when you come to your appointment:

  • Primary Identification - A current government issued photo ID such as a drivers license, state identification card, US passport or military ID
  • Secondary Identification - Most often this is a social security card, voter information card, birth certificate, marriage certificate or US passport. 
  • A background screening request form from the agency requesting the screening. Your case manager or another agency contact should have given you this form.
  • If you do not bring your social security card, you will need to know your social security number to complete the paperwork.

If your address on your photo ID is not correct, we will need two of the following with your current address:

  • Utility bill
  • Voter information card
  • Vehicle registration or title
  • Paycheck stub with name and address
  • Public assistance card (such as medicaid)
  • Spouse/parent affidavit
  • Canceled check or bank statement
  • Apartment or home lease
  • Mortgage documents
