This is a professional position responsible for providing the support, technical and clinical assistance to post adoptive families.

- Establish and maintain positive working relationship with HFC customers
- Provide post adoption services to families including linkage to mental health services, adoption support groups, resources in the community; including post adoption services covered by Medicaid
- Participates in and/or facilitates meetings, family team conferencing, multi-disciplinary, reunification, permanency, adoption and post adoptions staffings, and presentations promoting and supporting adoption including compiling post adoption data for end-of-the year reports/presentations and COA
- Evaluates the rationale for services, assessments of family needs and need for additional evaluations through the consideration of the child/family’s circumstance and the provider’s ability to meet the service need or provide alternate settings
- Create, update and maintain up-to-date tracking systems (FSFN, excel spreadsheets, etc.) including completing the mandatory one-year post adoption communication contacts within the mandated timeframe and timely creating/updating the post adoption tab each time post adoption services are provided to families
- Review and provide post adoption support/services to families involved in new intake (abuse) reports and Department of Juvenile Justice
- Verify, complete and provide tuition waiver forms and proof of subsidy letters
- Complete enhance subsidy requests
- Compete referrals to Agency with Persons with Disabilities
- Assist adoptive families with Medicaid related concerns
- Explore community supports in Circuit and neighboring counties to develop/maintain valuable supports/services
- Meet with community partners to increase knowledge on services and referral processes
- Attend adoption support groups, specialty support groups, adoption hearings
- Co-Facilitate and participate in Adoption Informational Night and Adoption 101 Classes; assist with planning of adoption events and community events
- Perform special projects as assigned

Bachelor’s Degree in Social Work or Human Services field, a minimum of three years’ experience in child protection and/or child welfare and clinical experience required.

Child Protection Professional preferred and child welfare eligibility determination and knowledge FSFN experince preferred.
