Staffing Facilitator

  • Job Summary:

    This is a professional position responsible for facilitating the transfer of dependency cases from the Department of Children and Families to Heartland for Children’s subcontracted case management organizations and providing the operational oversight needed to ensure children achieve timely permanency.

  • Essential Functions:

    • Establish and maintain positive working relationships with HFC internal and external customers
    • Facilitate case transfer staffings and  ensure cases are appropriately transitioned to HFC’s Case Management Organizations
    • Work collaboratively with DCF and CMO’s to ensure decisions are made with fidelity to the Safety Decision Making Methodology model
    • Establish ongoing communication with CMO leadership including permanency managers as it relates to the identification of cases with barriers to timely permanency
    • Work collaboratively with internal HFC departments and CMO’s to ensure  permanency and placement decisions are made to support child safety and is aligned with the mission, vision and values of HFC
    • Champion assigned performance measures or tasks to improve permanency outcomes and performance.
    • Champion licensed care and residential group care step down activities 
    • Analyze HFC/DCF data, scorecard measures and report to evaluate agency compliance with outcomes for children in order to target challenge areas requiring technical assistance and oversight
    • Review and analyze cases, identify barriers to permanency and recommend action necessary to bring permanency to children.
    • Observe and evaluate current case management permanency processes to ensure compliance with statutory and contractual requirements
    • Provide technical assistance and guidance to the case management agencies and other key stakeholders with regard to permanency, policy and procedure in determining and securing appropriated services as needed
    • Identify trends, good practice and barriers related to the permanency process and make written recommendations on a quarterly basis for improvement and/or training opportunities
    • Perform special projects as assigned

  • Qualifications Required:

    Bachelor's degree in social services or related field required.  Minimum of three years' experience in child protection and/or child welfare; demonstrated professional application of social work theories and methods in a child protection or child welfare environment. 

  • Qualifications Preferred:

    Master's degree preferred.
