  • Job Summary:

    This is a professional position responsible for processing intake and placement referrals, assess client needs, place foster children in the least restrictive setting and coordinate services as appropriate within HFC or with Case Management and or with Child Protective Investigators.  This position requires the ability to utilize time management skills, prioritize tasks, display positive/appropriate customer service, resolve conflicts/problems/issues and function on an independent basis, requiring minimal guidance/supervision.

     

  • Essential Functions:

    • Establishes and maintains positive working relationship with HFC customers
    • Receive referrals from DCF and Case Management Organizations and ensures that all necessary information for adequate intake is received
    • Ensures timely entry, review and tracking of information as inputted into the Care Match and other required systems such as Florida Safe Families Network (FSFN)
    • Receives and evaluates all incoming referral calls for foster care placements
    • Coordinates, in conjunction with Case Management or Child Protective Investigators admission and discharge process for clients
    • Assess presenting information for risk and acuity of each child and family member
    • Reviews/interviews potential admissions for appropriateness to facility
    • Provide on-call 24 hour/7 day coverage for foster care intake and placement services
    • Collect assessment information from available records
    • Maintain knowledge of Circuit 10 community resources
    • Attends necessary staffings that pertain to foster care Intake/Placement
    • Maintain awareness of children in foster care who may need to be reviewed for services as provided specifically for victims of CSEC and Human Trafficking; participate in staffings and document the outcome and efforts to link with such services
    • Cross-trains co-workers in the intake/placement process/department
    • Perform special projects as assigned

  • Qualifications Required:

    Bachelors degree in Social Services or related field

  • Qualifications Preferred:

    Experience in child welfare assessment and crisis intervention; experience in interviewing and negotiating preferred.
