  • Job Summary:

    This is a professional, licensed position that will ensure the primary care service coordination for dependent children in care.   Will be responsible for the Child Welfare Specialty Plan, 72-hour well-child checkups, monitor medical metrics and follow high risk/high need children.

  • Essential Functions:

    • Establishes and maintains positive working relationship with HFC internal and external customers

    • Assists in the coordination of medical care for children identified as high risk and who is placed in therapeutic group or residential care due to Intrastate movement

    • Knowledgeable of case management and care coordination services

    • Ensures that an initial Health Risk Assessment is completed and submitted to Sunshine Health

    • Schedules and manages staffings for children who have been assessed, recommended or placed in medical foster care, deemed medically complex or fragile, who have ongoing medical or dental needs, which impact their care while in the dependency system.

    • Participate on conference call meetings, attend and facilitate trainings.

    • Report any identified issues to child welfare case management, Sunshine Health or health care plan and places appropriate referrals for medical and behavioral case management

    • Monitors immunization status and/or development of “catch up” immunization schedule for dependent children in collaboration with the caregiver and in accordance with the American Academy of Pediatrics periodicity schedule

    • Monitors medication and reconciliation data to ensure compliance with prescription orders, including psychotropic medication

    • Assists as needed with attainment of 5339 form for psychotropic medication consent

    • Follows up with case manager and/or caregiver following all emergency admissions and document accordingly into Integrate system via IMV notes regarding discharge follow up recommendations

    • Monitor enrollment status of children overall and especiallyplaced out of county or state

    • Alert Resource Specialist when children on SSI enter into the system of care and are eligible for renewal of benefits

    • Educate young adults in extended foster care with understanding their plan options and benefits

    • Process and submit Care Grants, ensuring that application is complete with accompanying documentation

    • Coordinating with REVMAX in regards to appropriate demographic changes

    • Work closely with HFC HEDIS point of contact

    • Serves as a consultant to case management on making timely, appropriate and effective referrals to medical care services

    • Assist CMS caregivers in obtaining Durable Medical Equipment, locating appropriate providers and specialists. Participates in discharge planning and coordination with health care plan from inpatient services including the monitoring of post-discharge follow-up appointments, ordered services, durable medical equipment and ensures that medical prescription medications are filled for all kids in foster care.

    • Reviews the health and wellness reports, FSFN reports and other data sources for status of required annual physical, dental and vision screenings for children placed in out of home care and coordinate with child welfare case manager and/or caregivers as needed to ensure scheduling of needed appointments.

    • Perform special projects as assigned

     

  • Qualifications Required:

    Licensed registered nurse.

  • Qualifications Preferred:

    Minimum of two years of nursing experience preferably in the child welfare or behavioral health care setting.
