  • Job Summary:

    This position is responsible for delivering family support services to relative and non-relative caregivers by utilizing a broad range of community services, resources and natural support systems. Will be responsible for training and development, assessing services and consultation to families.

  • Essential Functions:

    • Establish and maintain positive working relationships with HFC internal and external customers
    • Maintains a caseload of families to provide weekly contacts, home visits, office visits, telephone or correspondence
    • Completes DCF/ACCESS applications with families to seek public benefits (cash assistance, food stamps and Medicaid)
    • Develops and maintains a list of individuals, organizations and providers as resources to caregivers
    • Works with caregiver support specialists to support placement instability situations
    • Invites caregivers to support groups, trainings and events
    • Coordinate annual caregiver support appreciation events
    • Facilitates support group for relative caretakers regularly
    • Assists families in developing and accomplishing support plan goals
    • Supports children living with relative caregivers to remain outside of foster care
    • Creates, implements and evaluates effectiveness of each family’s support plans
    • Timely completes assessments, program statistics and other required documents
    • Ensures families have follow-up linkages into the community
    • Follows-up to ensure family has benefits in place
    • Participate in quality reviews of the program services, outcomes and maintenance of records
    • Perform special projects as assigned

  • Qualifications Required:

    Bachelors degree in Social Work or related field. 

  • Qualifications Preferred:

    Florida Child Welfare Certification preferred.
